The P226 X-Five Extreme carries Sig's Custom Works reputation, and owners of the X-Five line generally describe the fit and trigger as tighter than a standard P226, with a crisp single-action break that competition shooters favor. Reliability feedback is largely positive, though some owners note the tight tolerances mean the slide benefits from a proper break-in before it runs at its smoothest.

The G10 grips and full-size metal frame give the pistol a substantial, steady feel in the hand, which owners like for range and competition use, though some find the added weight less ideal for daily carry. Pairing the Romeo-X optic with XRAY3 sights as a backup is popular among owners who want a fast sight picture without giving up iron sights as a fallback.

Buyers view the X-Five Extreme as a premium purchase within the P226 lineup, with the included optic and three magazines seen as good value against buying those parts separately. The SIG-LOC footprint and the wide P226 aftermarket base mean owners generally find grips, sights, and holsters easy to source.
